These Regulations may be cited as the Registration of Births, Still-births, Deaths and Marriages (Prescription of Forms) (Scotland) Amendment Regulations 1994 and shall come into force on 1st January 1995.

In these Regulations, “the principal regulations” means the Registration of Births, Still-births, Deaths and Marriages (Prescription of Forms) (Scotland) Regulations 1965.

For regulation 16 of the principal regulations there shall be substituted the following regulation:—“
16 
The abbreviated certificate of birth based on information contained in the Adopted Children Register or the Parental Order Register and issued under section 40(2) of the Act shall be in the form or as nearly as may be in the form set out in Schedule 16 to these regulations.”.

For the form set out in Schedule 16 to the principal regulations, there shall be substituted the form set out in Schedule 1 to these regulations.
5 
For the form set out in Schedule 23 to the principal regulations there shall be substituted the form set out in Schedule 2 to these regulations.
6 
For the form set out in Schedule 24 to the principal regulations there shall be substituted the form set out in Schedule 3 to these regulations.
7 
Regulation 7 of and Schedule 5 to the Registration of Births, Still-births, Deaths and Marriages (Prescription of Forms) (Scotland) Amendment Regulations 1971 are hereby revoked.
